4-H Canada COVID-19 Vaccination Declaration (Attestation) and Verbal Notice

The purpose of this 4-H Canada form is to provide a sign-off document confirming full COVID-19 vaccination.  In the case of a club meeting or repetitive event, the document is only to be completed ONCE like the COVID-19 Waiver and will be stored by 4-H Manitoba.  The Verbal Notice is for volunteers to read at the start of each in-person event (indoor/outdoor).  Attendance at the in-person event means you have completed the Declaration Form.  Please click here for Vaccination Declaration and here for Verbal Notice.
